PROFORMA OF AN APPLICATION FOR PEE QUOTA BY A SMALL-SCALE POWERLOOM UNIT
(to be prepared in type-print, without any correction by hand )

2. Address/addresses where the looms covered : by the application are installed by the applicant and family members.

3. Whether applying for quota for fabrics : (cloth)/made-ups of cotton/synthetic (state here clearly).

4. Details of the attached powerloom permit/ : acknowledgement obtained under the T(C)O/T(DR)O and covered in the application (as mentioned at para 3(ii) of the memorandum) (to be given below)

5. Has the applicant attached a valid RCMC : issued by the PDEXCIL in the case of cotton textiles categories/issued by the SRTEPC in the case of man-made textiles categories. If so, the page no. of the application at which attached.

6. Has the applicant attached an affidavit in the : prescribed form on stamp paper of Rs. 10/- signed by himself to the effect that the exports under the quota allotted to him shall be made as per the guidelines enunciated in the Memorandum? If so, indicate the page no. of the application at which it is attached. (please note that the name of the applicant in the application, powerloom permit/ acknowledgement, RCMC and the affidavit should be the same. If it is different, the reason thereof should be clearly stated)

7. Has the choice of (seven) country-categories : combination (list to be submitted in triplicate, in separate sheets) in the order of preference, clearly indicating whether the items are cotton/ synthetics items, been attached. If so, indicate the page no. of the application which is attached.

8. Import-Export (I.E.) code no. : The contents of this application are true and correct to the best of my /our information and knowledge. If any of the aversions/facts stated here in above or in the accompanying documents are found to be incorrect or untrue, I/We understand that this application will stand rejected.
